Declarations - Conditions in relation to being one of our Community Champions - Please read all carefully!

You must not share details of any of our services users with anyone without our (Community Cares Club) consent.

2 *

You must not take pictures of our service users.

3 *

You do not make video recordings of our service users, this includes audios.

4 *

You must disclose to us (Community Cares Club) immediately any evidence of abuse you perceive during the period you are with any of our (Community Cares Club) service users.

5 *

You must not mention or disclose information about our service users on any social media platform.

6 *

You must share with other professionals and governing bodies any information that you are aware of, in order to promote the well-being of and to protect our service users from any form of abuse. This must be done with our (Community Cares Club) awareness.

If you are unable to pick up or deliver meals to any of our (Community Cares Club) service users, you must try as much as possible to contact any of our (Community cares Club) coordinators before returning to base.

8 *

You (Our Community Champion) must immediately report to us (Community Cares Club) coordinators, any form of abuse or discrimination you may have experienced whilst supporting our (Community Cares Club) service users.

9 *

You (Our Community Champion) must not send information relating to our service users to us (Community Cares Club) via any social media platforms.

10 *

We (Community Cares Club) reserve the right to carry out, when necessary, a criminal history check on our community champions. This is to protect our service users and other community champions.

11 *

You (Our Community Champion) must disclose any criminal convictions you carry (except driving related offences) to us (Community Cares Club).

12 *

We hope you, (Our Community Champion) enjoy every second of your time with us and with our service users, therefore we encourage that you share with us any concerns you may have about our service users or other Community Champions.

This will help us to ensure everyone is safe and well protected.

You (Our Community Champion) must inform us of any changes to your contact details.

14 *

We (Community Cares Club) will not share your information with anyone except in cases of emergency and/or as part of any investigation relating to us (Community cares Club) and/or any of our service users to the authorises or other professional bodies.

You (Our Community Champion) must provide us (Community Cares Club) with details of who to contact in case of an emergency that may occur whilst you are offering your services to us (Community Cares Club).

We (Community Cares Club) are a registered not-for-profit organisation and do not promise neither guarantee to offer you paid employment. However, if the opportunity does arise, you (Our Community Champion) may be considered.

You (Community Champion) must not give any of our service user your personal number. We advise that in the event that you need to call that you add 141 to the start of the number, this will withhold your number. This is for your own safety & well-being
